A compact smooth <cobordism> is an <h-cobordism> when both boundary inclusions $M_i\hookrightarrow W$ are <homotopy equivalences>. The name does not itself impose simple connectivity. A <trivial cobordism> is an <h-cobordism> because a product cylinder retracts onto either end.
