= Solution

For a two-dimensional incompressible flow, the <stream function> convention
$$
u_r=\frac1r\frac{\partial\psi}{\partial\theta},
\qquad
u_\theta=-\frac{\partial\psi}{\partial r}
$$
is satisfied by
$$
\boxed{\psi(r,\theta)=\frac{d\sin\theta}{r}+C}.
$$
