Most dosing mistakes with lyophilized peptides are arithmetic mistakes, not medical ones. A vial is labeled in milligrams, bacteriostatic water is added in milliliters, and the syringe is marked in insulin units. Three different units, one injection.
The core formula is simple. Concentration equals peptide mass divided by diluent volume. Put 2 mL of bacteriostatic water into a 5 mg vial and you have 2.5 mg/mL. A U-100 insulin syringe has 100 units per mL, so each unit holds 0.025 mg, or 25 mcg. A 250 mcg dose is therefore 10 units on the barrel. Reconstitution notes worth keeping. Aim the water down the inside wall of the vial rather than directly onto the powder, and swirl instead of shaking. Choose a diluent volume that puts your typical dose somewhere between 5 and 20 units, since very small unit counts magnify measurement error. Record the exact volume you used, because a vial reconstituted differently next month will not match your old unit counts.
Titration and timing. GLP-1 protocols usually step up on a fixed weekly cadence, and holding a dose for an extra week is common when side effects are heavy. Semaglutide has a half-life near 7 days, which is why weekly dosing produces relatively flat levels and why a missed dose is not immediately obvious. Tirzepatide sits around 5 days. Knowing the half-life tells you how long a change actually takes to show up.
Injection sites. Rotate between abdomen, thigh and upper arm, and stay at least one inch from the previous site and from the navel. Logging the site alongside the dose is the only reliable way to keep rotation honest over months.
