Cardiac vein 2026-11-05
A cardiac vein carries blood away from the heart's own tissue after passage through its supplying vessels. This is the return side of the coronary circulation, distinct from the large volume of blood passing through the ventricular cavities. Much of this return reaches the right atrium through the coronary sinus; anterior cardiac veins can return directly to the right atrium.[ref]
The great cardiac vein, middle cardiac vein and small cardiac vein provide named routes through this network. An illuminating anatomical exercise pairs each route with its connections rather than simply matching it to the nearest coronary artery. The artery supplies tissue and the vein drains tissue, while their branching and crossings need not be identical.
Primary evidence for that qualification comes from a study dissecting 37 fixed human hearts. It documented varying relationships among the coronary sinus, its tributaries and neighboring arteries.[ref]
The anatomy of the coronary sinus and its tributaries by J. R. Ortale and others. Abstract, methods and vessel relationships
Such a study observes actual connected vessels in specimens; a schematic drawing deliberately compresses the variation into a readable route map.For a transport measurement, identifying the correct cardiac vein matters because a regional sample has a regional venous oxygen content. Substituting it for mixed venous oxygen content in a whole-body Fick principle calculation would change the system being sampled. The anatomical return route and the corresponding blood flow must match the oxygen-balance question.
Coronary circulation 2026-11-05
The coronary circulation supplies and drains the heart's own tissue. Coronary arteries branch from the aorta to supply the myocardium; cardiac veins return blood toward the right atrium.[ref] Blood moving through a ventricular cavity is not the same flow as blood perfusing the surrounding muscle.
Much coronary venous return passes through the coronary sinus, while anterior cardiac veins provide a direct route to the right atrium.[ref] Consequently a coronary-sinus sample represents its drainage territory rather than automatically every part of the heart. Anatomical pathways must match the region whose blood flow and oxygen uptake are being estimated.
An original tissue-balance example uses regional flow , arterial oxygen content and matching venous oxygen content in the same units. Then regional coronary oxygen uptake isThe arterial delivery rate is in those units. Delivery and uptake differ because oxygen remains in the returning blood. The values are stipulated, not normal-reference limits.
A primary 2007 experiment measured coronary volumetric flow using continuous saline thermodilution, an infusion catheter and a pressure/temperature guidewire, with comparisons in dogs and patients.[ref]
Direct volumetric blood flow measurement in coronary arteries by thermodilution by Wilbert Aarnoudse and others. Abstract, Methods
This provides a concrete way to investigate the flow term; it does not by itself measure the oxygen-content difference. The arterial map also varies among hearts. A radiopaque-injection study classified 178 of 200 examined hearts as right dominant by the origin of the posterior interventricular artery.[ref]
Functional dominance of the right coronary artery: incidence in the human heart by C. E. Nerantzis and others. Abstract
Dominance names an anatomical branching criterion, not a claim that one artery supplies the entire heart. Coronary sinus 2026-11-05
The coronary sinus is a major venous collecting channel of the heart that opens into the right atrium. It receives blood from cardiac veins, including the great cardiac vein, middle cardiac vein and small cardiac vein.[ref]
Anatomy and Physiology 2e by J. Gordon Betts and others. Section 19.1, Coronary Veins and Figure 19.15
It belongs to the venous return pathway, distinct from the coronary arteries supplying the myocardium. The collecting-channel arrangement explains its use in a regional balance. Incoming flows combine before the sample or outflow observation is taken. In a steady model with negligible local storage,If represented inflows are and , the sinus outflow is . This is a chosen continuity example, not a normal flow value.
A measurement at the coronary sinus does not automatically represent every myocardial route. If another modeled pathway returns directly to a chamber, the total venous return from the represented heart is , while the sinus observation remains . Treating the latter as the whole total would miss about in this example.
Likewise, venous oxygen content sampled from a collecting channel is a mixture weighted by its contributing flows. It should be paired with flow and arterial measurements representing compatible tissue when calculating coronary oxygen uptake. Knowing the sampling site is part of defining the biological quantity, not simply a label attached after measurement.
Left marginal branch 2026-11-05
The left marginal branch is a branch of the circumflex artery running toward the left ventricular margin and supplying part of the left ventricle. It belongs to the arterial supply of the heart's tissue, rather than to the blood flow ejected from the ventricular cavity.[ref]
Anatomy and Physiology 2e by J. Gordon Betts and others. §19.1, Coronary Circulation, Figure 19.15 and description of the circumflex marginal territory
The parent connection is important: left identifies the relevant side, and marginal describes a course near a ventricular margin. A surface vessel in that region should not be identified solely by proximity. Tracing its origin from the circumflex artery distinguishes an arterial branch from a nearby cardiac vein and from branches of the right coronary artery.
Selective contrast injection provides an experimental way to examine such branching. A primary 1967 report described preformed catheters for selective coronary angiography, with serial or cine imaging in 100 consecutive patients.[ref]
Selective Coronary Arteriography: Part I: A Percutaneous Transfemoral Technic by Melvin P. Judkins. Abstract, apparatus and patient series
The resulting image reveals a filled vascular route; it does not directly measure how much tissue oxygen was consumed along that route.A simple branch balance illustrates the additional information needed. If total blood flow entering a segment of the circumflex artery is 100 mL/min and 30 mL/min enters one left marginal branch, the remaining downstream branches receive 70 mL/min in a no-storage model. The parent flow alone cannot determine the split. Vessel geometry, vascular resistance and the measurement location all affect what a branch-flow observation means.
Middle cardiac vein 2026-11-05
The middle cardiac vein follows the posterior interventricular sulcus and drains into the coronary sinus. Its course is associated with the posterior descending artery on the posterior/inferior surface of the heart.[ref] It is a cardiac vein, carrying return from heart tissue rather than serving as an outlet from a ventricular cavity.
For specimen orientation, first identify the posterior interventricular sulcus, then trace the middle cardiac vein toward its junction with the coronary sinus. Tracing the neighboring posterior descending artery instead leads back toward an arterial source. The same surface corridor thus supports two different paths through the coronary circulation.
Connections can also cross the simplified boundaries of a schematic drainage map. A primary dissection study reported an approximately 1 mm connection between the anterior and posterior interventricular venous routes in of its 37 specimens.[ref]
The anatomy of the coronary sinus and its tributaries by J. R. Ortale and others. Abstract, interventricular venous anastomosis
This observation illustrates a network property: drawing separate named tributaries does not prove that they are isolated tubes. For an oxygen-balance experiment using a venous sample, the actual drainage connections determine which tissue contributions can reach the sample. Neither the vessel's name nor its surface position alone specifies an exact sampled tissue volume.
