There are two primal inequalities, so there are two nonnegative dual variables. The five primal variables yield five dual inequalities:
minimise 2y1+y2,y1+y2≥1,−y1+y2≥−1,−y1+3y2≥−5,3y1+4y2≥12,3y1+y2≥6,y1,y2≥0.

Each coefficient column of the primal linear program becomes one row of this dual linear program.
