A measurement channel writes a POVM's outcome into an orthogonal classical register:
Φ(X)=∑a​Tr(Ea​X)∣a⟩⟨a∣.

For any input orthonormal basis ∣j⟩, the Kraus operators Ka,j​=∣a⟩⟨j∣Ea​​ realize this map and satisfy ∑a,j​Ka,j†​Ka,j​=I. It is therefore a trace-preserving completely positive map.
