Choose the four unit Bloch vectors at the vertices of a regular tetrahedron, so ∑ini=0 and ∑ininiT=4I/3. The displayed four positive rank-one effects sum to I. On input (I+r⋅σ)/2, their probabilities are pi=(1+r⋅ni)/4, giving the reconstruction r=3∑ipini. This realizes the smallest fixed qubit informationally complete POVM.
